Try to keep all brackets and commas so it is easier for me to copy/paste.

The layout view is 10 rows, with 12 numbers in each row, separated by a comma. It is viewed from above, just as you would be looking down at it.

 It is okay if there is a space after a number, so long as there is a comma. Also the numbers do not have to conform to exact column, as you can see in this example, there are some 3 digit numbers. Use 0-127. 

Replace question marks with your channel number. (channel 1-16)
